La Diagnosi Genetica Preimpianto \u00e8 una metodica diagnostica prenatale che viene eseguita sull’embrione prima del suo trasferimento. Consiste nell’esame genetico delle sue cellule per sapere se portano qualche alterazione cromosomica o genetica e selezionare solo embrioni sani da trasferire nell’utero.<\/p>\n
\u00c8 un processo complesso che non pu\u00f2 essere eseguito in tutte le malattie. Nei casi ereditari, la Legge regola in quali malattie dei genitori si pu\u00f2 eseguire la PGD, e quando non \u00e8 possibile o bisogna chiedere l’autorizzazione alla Commissione Nazionale per la Riproduzione Umana.<\/p>\n

La PGD \u00e8 indicata quando vi \u00e8 il rischio che gli embrioni presentino gravi alterazioni nel numero o nella costituzione dei cromosomi, un’alterazione del gene che causa una malattia genetica ereditaria, o quando esistono dubbi sulla fattibilit\u00e0 degli embrioni.<\/p>\n
Per eseguire questo trattamento copriamo quattro fasi.<\/p>\n

In questa fase effettuiamo test di caratterizzazione genetica delle malattie da diagnosticare nei genitori portatori. Lo scopo \u00e8 quello di avere quante pi\u00f9 informazioni possibili prima dell’applicazione del DGP.<\/p>\n

Con tutte le informazioni necessarie gi\u00e0 raccolte, e nonostante il fatto che la coppia possa non avere problemi riproduttivi, per una maggiore sicurezza si procede all’ottenimento degli embrioni tramite trattamento di fecondazione in vitro.<\/p>\n

La biopsia dell’embrione pu\u00f2 essere eseguita in due fasi:<\/p>\n
1) Il terzo giorno dopo la fecondazione, quando il pre-embrione \u00e8 allo stadio di 6-8 cellule, ne estraiamo uno o due senza comprometterne lo sviluppo.<\/p>\n
2) Il giorno 5 o 6 dopo la fecondazione, quando il pre-embrione \u00e8 allo stadio di blastocisti, estraiamo diverse cellule dal trofoectoderma. In questo caso e per il tempo necessario per ottenere i risultati, gli embrioni devono essere congelati una volta sottoposti a biopsia.<\/p>\n
Dopo la biopsia, il pre-embrione viene tenuto in coltura o congelato fino a quando non si ottiene il risultato della diagnosi e si valuta il suo eventuale trasferimento. La biopsia ottenuta viene inviata ad un laboratorio di genetica di riferimento, dove sar\u00e0 processata mediante tecniche CGH Array, PCR o NGS (sequenziamento di massa di nuova generazione). In questo modo otteniamo uno studio e una diagnosi di ogni embrione sottoposto a biopsia.<\/p>\n
