What to do when spontaneous miscarriage becomes a pattern"},"content":{"rendered":"<p>A <strong>spontaneous miscarriage<\/strong>s\u00a0is one of the most common complications of pregnancy and occurs\u00a0<strong>in 10 to 15% of clinical pregnancies\u00a0<\/strong>(pregnancies which we\u2019ve initially been able to visualise on an ultrasound). Miscarriages can occur whether the pregnancy was conceived naturally or through assisted reproduction treatment.<\/p>\n<p>It is considered to be a<strong>\u00a0spontaneous miscarriage when the pregnancy is lost before week\u00a020<\/strong>\u00a0of gestation. This can be early (if it happens before 12 weeks), or late (if it\u2019s after 12 weeks). The majority of miscarriages, however, usually occur before week 10 of pregnancy.<\/p>\n<p>At our fertility clinic in Spain it is considered\u00a0to be recurrent or habitual miscarriage<strong>\u00a0when a female miscarries spontaneously two or three times in a row<\/strong>. This situation, which is uncommon, occurs in\u00a01% (women with more than three consecutive miscarriages), and\u00a05% (more than two consecutive miscarriages), of the population. After two miscarriages in a row, the risk of miscarrying again is approximately 20%. After a third pregnancy loss, this risk increases to\u00a030%. Both types of causes may be sporadic or permanent.<\/p>\n<p>In addition,\u00a0other causes of miscarriage are associated with<strong>\u00a0genetic problems and chromosome issues in the embryo<\/strong>,\u00a0<a href=\"https:\/\/urecentrogutenberg.es\/en\/becoming-a-mother-at-40-pregnancy-risks-beginning-at-this-age\/\" target=\"_blank\" rel=\"noopener noreferrer\">maternal age<\/a>, thrombophilia (acquired or hereditary hypercoagulation), such as\u00a0antiphospholipid syndrome, or genetic mutations on normal blood clotting factors. Endocrine disorders that are poorly controlled such as diabetes mellitus, immune conditions, abnormalities in uterine anatomy or abnormalities in the sperm or its quality may also be the reason behind spontaneous miscarriage.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"How_do_we_help_couples_who_are_dealing_with_recurrent_spontaneous_miscarriage\"><\/span>How do we help couples who are dealing with recurrent spontaneous miscarriage?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>At our fertility clinic in Spain, when talking about a female with a history of two or more spontaneous miscarriages, whether she conceived naturally or through assisted reproduction treatment,\u00a0<strong>we re-evaluate the couple and go over each of the possible causes<\/strong>, both at the embryo and maternal level.<\/p>\n<p>To try to find a cause\u00a0<strong>we carry out a complete, thorough study on the couple<\/strong>\u00a0in which, among other tests, we will order karyotype testing for both parents. To rule out\u00a0<a href=\"https:\/\/urecentrogutenberg.es\/en\/how-to-interpret-a-sperm-analysis\/\" target=\"_blank\" rel=\"noopener noreferrer\">sperm issues<\/a>\u00a0we can perform an analysis called sperm FISH\u00a0and a sperm\u00a0DNA fragmentation test. \u00a0Additionally, we will assess the uterine cavity with a\u00a0<a href=\"https:\/\/urecentrogutenberg.es\/en\/diagnostic-hysteroscopy-in-assisted-reproduction\/\" target=\"_blank\" rel=\"noopener noreferrer\">hysterosonogram and\/or a hysteroscope<\/a>\u00a0which will allow us to rule out malformations, chronic inflammation, polyps and uterine fibroids, etc.<\/p>\n<p>Another study that can provide us with more information about these recurrent miscarriages is the presence of the most frequent thrombophilias, which we can study with a simple blood test. We should also\u00a0<strong>rule out endocrine and metabolic problems\u00a0<\/strong>that are not being controlled. If possible, we should even perform a chromosome analysis on the embryo tissue recovered from the products of pregnancy (after the D&amp;C is done), trying to avoid contamination with maternal tissue as best as possible, as this could give us incorrect information.<\/p>\n<p>In terms of possible psychological causes in these situations, though it\u2019s been proven that there is no cause and effect relationship between psychological\u00a0<strong>stress and recurrent miscarriage<\/strong>, at URE Centro Gutenberg\u00a0we believe it is essential for\u00a0couples\u00a0<a href=\"https:\/\/urecentrogutenberg.es\/en\/tools-for-facing-infertility\/\" target=\"_blank\" rel=\"noopener noreferrer\">to get psychological counselling<\/a>\u00a0if they\u2019ve received this diagnosis.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"Is_it_always_possible_to_determine_the_cause_of_a_spontaneous_miscarriage\"><\/span>Is it always possible to determine the cause of a spontaneous miscarriage?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Unfortunately, despite all of the testing and studies we\u2019ve mentioned, and even other analyses which could be performed,\u00a0<strong>between 50\u00a0and\u00a075% of couples who miscarry won\u2019t be diagnosed with a specific cause,\u00a0<\/strong>and this is known as idiopathic recurrent miscarriage.<\/p>\n<p>It is, however, important to keep in mind that, like with everything in assisted reproduction, the\u00a0<a href=\"https:\/\/urecentrogutenberg.es\/en\/how-a-females-age-impacts-her-fertility-and-chances-of-becoming-a-mother\/\" target=\"_blank\" rel=\"noopener noreferrer\">female\u2019s age is one of the main prognostic factors<\/a>.<\/p>\n<p>Therapeutic or treatment alternatives in this situation<strong>\u00a0will depend on whether the cause is unknown\u00a0<\/strong>or, on the contrary, if we have determined a condition or reason that is triggering recurrent pregnancy loss in the female.<\/p>\n<p>At present there are many studies and developments in reproductive immunology, endocrinology, genetics\u2026 meaning we are able to produce<strong>\u00a0diagnoses that are increasingly more precise and accurate<\/strong>, which would require more specific therapeutic interventions.
